Olympia HSA Basic

?         Annual fee of $249 - with no administration fees

?         Claim limit is $15,000 annually & includes a spouse and dependents. Maximum is 2 employees. 2nd employee must be a spouse or equivalent  

?         All claims processed online or using the claims app - Android / iPhone

?         Attached is a list of eligible and ineligible expenses

Olympia HSA Plus

?         Annual fee of $499 - with no administration fees

?         Claim limit is $15,000 annually - max 2 employees. 2^nd employee must be a spouse or equivalent

?         Travel Insurance, Emergency Medical Insurance and Catastrophic and Drug Coverage are included - see page 3 of the attachment HSA Plus

?         All claims processed online or using the claims app - Android / iPhone

?         Same list of eligible and ineligible expenses as with HSA Basic above

Olympia HSA Deluxe

?         Annual fee of $699 - with no administration fees

?         Claim limit is $15,000 annually - max 2 employees. 2^nd employee must be a spouse or equivalent

?         Travel Insurance, Emergency Medical Insurance and Catastrophic and Drug Coverage are included - see page 4 of the attachment HSA Deluxe

?        Telemedical Dialogue - telemedicine for you and your employees Dialogue telemedicine for you and employees  &  https://www.dialogue.co/en/ultimateguide-telemedicine
